Vaccine Appointment Page

We are now scheduling appointments for the new Bivalent vaccine boosters. These updated boosters are more effective against the newer Covid variants and you can receive them at least two months after your last dose. They are now approved for anyone over 6 months. All Covid vaccines are free of charge and you do not have to be our patient to sign up

We are also accepting Flu Shot appointments through this website (For Flu Shots, if we are not in-network with your insurance or if you have an HMO plan, the cost is $30.)

Please select the appropriate appointment type based on the age and dose number. 

Once you book an appointment, please fill out the entire intake form.  It will be required to confirm your appointment.
